Overview
phpMyAdahas is a fully customizable online survey application built on XHTML, CSS, PHP and MySQL conforming to US 508 and W3C WAI standards. This allows to a create multiple paged questionnaire with different type of answering options.

These are the list of features available in the current version.

- Written in XHTML 1.1 markup and served as application/xhtml+xml (if browser is supported).
- Conforms to US 508 and W3C WAI standards.
Requirements
- PHP version 4.x.x
- MySQL version 4.x.x
- Any platform that the above two are supported
Installing
- Unzip the compressed archive to an empty directory.
- Open up config.inc.php with a text editor and set the URL and your database connection details.
- Save the file.
- Upload all the files to the web server.
- Open /setup in your browser. This will request for your confirmation to create/replace tables.
- Click create to create the tables, and when it’s complete, the username and password for the admin account will be displayed. Please note them down.
- Login to admin pages /admin from your browser and change the password immediately.
- That’s it!
